Thank you for your time & attention during Open Enrollment. Now that the Open Enrollment period has closed,
there are a few important reminders you should be aware of:

Dependent Documentation
If you added any dependents to your medical coverage during Open Enrollment, documentation is
required
to approve coverage. Please note ‐ documentation is due by 12/15/23.

SPOUSE: If your spouse is ELIGIBLE for medical coverage through his/her employer, he/she is NOT
eligible for medical coverage with Dupré. If your spouse is not eligible for medical
coverage through his/her employer and will be added to your medical coverage here, please
provide the two required documents below:
- Marriage Certificate

Select from the list below based on your spouse's employment status:
- A) If your spouse is unemployed or self-employed and you file taxes jointly,
please provide a copy of your 1040 (example here) ***OR***
- B) If your spouse is unemployed or self-employed and you do not file taxes
jointly, please turn in the "Spousal Insurance Inquiry Form" linked here
***OR***
- C) If your spouse is employed and not eligible for medical coverage through
his/her employer, please turn in the "Spousal Insurance Employer Inquiry Form"
linked here
- CHILD (up to 26 years old): Birth Certificate
- Documentation can be emailed to benefits@duprelogistics.com. You will receive
a response within
two business days. If you do not receive a response within this period of time, please resend
the document to avoid missing the deadline.

Evidence of Insurability / Statement of Health
If you enrolled in or made any changes to existing enrollment in Life Insurance, Short-Term
Disability, or Long-Term Disability, an Evidence of Insurability (EOI) is likely required. Your
coverage is pending until a completed EOI Form is received. Here's what you'll need to do:
- Fill out the EOI/SOH Form for each coverage that was changed/added
- Submit completed form(s) to Metlife via email ‐ SOHSubmissions@metlife.com
- If you have questions regarding the form or want to follow up on the status, please email
Metlife at eoi@metlifeservice.com or call
1-800-638-6420 Option 1 (Statement of Health unit).
- Once Metlife reviews your request for new/additional coverage, you will receive a letter in the
mail indicating if you've been approved.

New Benefits
As a reminder, your new benefits and premiums take effect on 1/1/24. If you are newly enrolled in
Medical coverage, Flexible Spending Account or Health Savings Account, you should receive your
card(s) in the mail soon.
Enrollees in Dental/Vision coverage do not receive insurance cards. The information you'll need can
be found in the Dupré Connect app under Benefits & Pay > Benefits > Health Benefits.
